Provisional License

As ofJuly 15, 2026, Kentucky now offers a Provisional License. ​​ Please make certain you are eligible for this license type before applying as all licensure fees are non-refundable. 

The requirements to be eligible for a provisional license are in KRS 311.571(8) and are as follows:

  • Fulfill the requirements for a regular license in KRS 311.571, subsection (1)(a), (b),(c), and (e) OR Fulfill the requirements for regular license in KRS 311.571​, subsection (2)(a), (b), (c), (d), and (e);
  • Have an offer for employment as a physician with a sponsor that is:
    • A professional practice, healthcare entity, or corporation that operates and is licensed or authorized to provide healthcare in the Commonwealth; and
    • Located in a medically underserved area as defined by the secretary of the United States Department of Health and Human Services;
  • Be in good standing or authorization to practice medicine with the medical licensing or regulatory institution of his or her licensing country within the last five (5) years;
  • Completed a residency or substantially similar postgraduate medical training; and
  • Practiced as a medical professional performing the duties of a physician in his or licensing country for no less than five (5) years. ​ ​

Application Instructions

​​This licensure type is not eligible for a temporary license and will require completion of a Federation Credentials Verification Service (FCVS) profile​. The deadline dates are available here​ and the application must be completed by the dates identified in the Special Licensure Items section. 

  1. Once you click on the application link, you will be required to create a login profile by choosing "Create An Account" on the on the top of page.
  2. Enter your name exactly as it appears on your social security card. The Department for Medicaid Services requires your name appear in our database exactly as it does on your social security card.
  3. After you have created a login profile, you have initialized your application process which consists of demographic information only.
  4. Select the option to apply for a Physician License.
  5. Upon completion of the login profile, you will be prompted to "Click Here To Apply For License."
  6. After completing the online application, you will be prompted for a $300 non-refundable application fee. Payment may be made by credit card or ACH debit.
  7. Once the payment is complete, you will be directed to your profile screen where you will see a pdf link for application instructions and forms.  These forms are required for your application to be considered complete. Please note these forms are fillable for your convenience.
  8. In addition to the regular pdf forms, you will be required to complete the forms attached here​.  
  9. Please be advised that it is the applicant's responsibility to verify eligiblity prior to applying for this license.
