The applicaiton process is approximately 60-90 days for all licenses offered by the Board, including the temporary permit.

The full Board meets quarterly, all licenses must receive full Board approval prior to issuance of a license with the exception of the temporary permit. Please see the Board Dates & Deadlines.

Permanent licenses are issued once the application is 100% complete and full Board approval is received.

Yes, the temporary permit is part of the regular medical/osteopathic license application and is offered to allow physicians whom are eligible the opportunity to practice prior to receiving approval by the full Board. Temporary permits are unrestricted and are issued for up to 6 months.

There is no separate fee for the temporary permit as is it part of the regular medical/ osteopathic license application and is covered under the $300 licensure fee.

Temporary permits are typically issued within 24 to 48 hours of eligibility.

Applications must be complete with the exception of the criminal background check and the certificate of completion of the HIV/AIDS education requirement, provided you have completed the Affidavit of Reasonable Cause with explanation.

Yes, the Kentucky Board of Medical Licensure has required the FCVS since March 1, 2005. The FCVS is required for the following licenses: Regular Medical/Osteopathic License, Residency Training License, Institutional Practice Limited License, and Faculty License.

No, it is not the policy of the Board to expedite one application over another. Applications are processed in the order they are received.

The NPDB/HIPDB is the National Practitioner Data Bank/ Healthcare Integrity and Protection Data Bank. A self-query is required of all applicants applying for a full medical/osteopathic license. Please go to the following website; click on “has a report been filed on you?” on the left under the title “practitioners”; click on “search on myself”; read the screen, scroll to the bottom of the screen, click in the box “I acknowledge and understand…”, then click on the “continue button”; you are now in the self-query and will need to complete as instructed. At the end of completing the online self-query where you will print a form that needs to be signed, notarized, and mailed directly to the NPDB/HIPDB. Once the NPDB has completed the report they will email you an electronic copy and mail you the original. You may submit either to the Board.
