Frequently Asked Questions For Physician Licensure

Q. What is the licensure fee for the Kentucky full medical/ osteopathic license?

A. The licensure fee is $300.

 

Q. How long is the application process?

A. The application process is approximately 60 – 90 days for all licenses offered by the Board, including the temporary permit.

 

Q. How often does the Board meet?

A. The full Board meets quarterly, all licenses must receive full Board approval prior to issuance of a license with the exception of the temporary permit. Q. When will my full medical/ osteopathic license be issued?

A. Permanent licenses are issued once the application is 100% complete and full Board approval is received.

 

Q. Does the Kentucky Medical Board offer a temporary permit?

A. Yes, the temporary permit is part of the regular medical/osteopathic license application and is offered to allow physicians whom are eligible the opportunity to practice prior to receiving approval by the full Board. Temporary permits are unrestricted and are issued for up to 6 months.

 

Q. Is there a fee for the temporary permit?

A.  There is no separate fee for the temporary permit as is it part of the regular medical/ osteopathic license application and is covered under the $300 licensure fee.

 

Q. When are temporary permits issued?

A. Temporary permits are typically issued within 24 to 48 hours of eligibility.

 

Q. What makes me eligible for a temporary permit?

A. Applications must be complete with the exception of the criminal background check and the certificate of completion of the HIV/AIDS education requirement, provided you have completed the Affidavit of Reasonable Cause with explanation.

 

Q. Is the FCVS a requirement?

A. Yes, the Kentucky Board of Medical Licensure has required the FCVS since March 1, 2005. The FCVS is required for the following licenses: Regular Medical/Osteopathic License, Residency Training License, Institutional Practice Limited License, and Faculty License.

 

Q. Does the Board expedite applications?

A. No, it is not the policy of the Board to expedite one application over another. Applications are processed in the order they are received.

 

Q. Can the Board expedite my FCVS packet process?

A. No, it is not the policy of the Board to request the FCVS expedite their process.

 

Q. Can I use a background check that has been sent to another state or entity?

A. No, you must have new fingerprinting done following the process in the instructions provided in your application pdf.

 

Q. What is the NPDB/HIPDB self-query report and how do I complete it?

A. The NPDB/HIPDB is the National Practitioner Data Bank/ Healthcare Integrity and Protection Data Bank. A self-query is required of all applicants applying for a full medical/osteopathic license.  Please go to the following website: http://www.npdb-hipdb.hrsa.gov/pract/hasAReportBeenFiledOnYou.jsp; click on “has a report been filed on you?” on the left under the title “practitioners”; click on “search on myself”; read the screen, scroll to the bottom of the screen, click in the box “I acknowledge and understand…”, then click on the “continue button”; you are now in the self-query and will need to complete as instructed. At the end of completing the online self-query where you will print a form that needs to be signed, notarized, and mailed directly to the NPDB/HIPDB. Once the NPDB has completed the report they will email you an electronic copy and mail you the original. You may submit either to the Board.

 

Q. What is the AMA/AOA profile and how do I complete it?

A. The AMA is the American Medical Association and the AOA is the American Osteopathic Association. An AMA physician profile is required of all applicants applying for a full medical license and an AOA physician profile is required of all applicants applying for a full osteopathic license. To complete the AMA physician profile go to: https://profiles.ama-assn.org/amaprofiles/ and choose the GO button next to the  ‘Physicians Only – Requests for Profiles to be Sent to Licensing Boards’ option; to complete the AOA physician profile go to: https://www.doprofiles.org/, go to the box titled ‘Physicians’ and choose ‘Send Your Profile.’ Your profile will be sent to us directly from the AMA or AOA.

Q. If I want to give permission for my information to be released to someone what do I need to do?

A. You may enter the person’s name in the appropriate box on the online application or you may submit a typewritten statement with your signature to the Board.

 

Q. I will not be working with patients; do I still need to complete the HIV/AIDS course requirement?

A. Yes, as of September 24, 1991, all applicants for medical licensure must comply with the two (2) hour AIDS education requirement. (KRS 214.615)
