Change of Address

Address changes to your mailing and practice address can be made online.  Please follow the directions below:

Before clicking on the link to change your address, please read the following instructions:

• To log on to the system, enter your KY license/certificate number as the username and the last four digits of your social security number as your password and click the log in button.
• If you are an applicant for licensure/certification and do not have a license, please use the username and password assigned to you by our licensing department.
• Once the screen loads, choose My Account on the left side of the page.
• Choose Addresses under the My Account tab.
• You will have the opportunity to change your mailing and practice address.  There is also an option to change your temporary license address.  The temporary license address is for new licensure applicants and does not apply to licensed physicians.
